Parent Permission

Consent to Prompt

In the classroom we use precision commands
  • Each child is given three directions “general”, “please”, and “need” direction to comply before being prompted.
  • Depending on your child’s abilities to follow directions, they may only receive a “general” or “please”
You will, and should have already, signed an informal consent to prompt form.
  • Formal form will be signed three weeks after data is collected.
  • This falls under a Level 3 in the Least Restrictive Behavioral Interventions Special Education Rules: Utah State Office of Education, as a forceful physical guidance.

Pictures

NUAP will occasionally use our student’s pictures in such things as flyers, learning skills, DVD, and training videos
  • We need your permission to use your child’s photo on these items
  • We ask that you please don’t take any pictures while volunteering in the classroom.
Pictures of my child may be used on:
  • NUAP flyer
  • Learning skills videos
  • NUAP staff training videos
  • NUAP informational video
  • Newspaper articles providing information about NUAP
  • Posters providing information about NUAP

Release Information

Authorizes the staff of NUAP/Weber Human Services to release information to the
  • School District
  • Speech services
  • Transition into kindergarten
You have the right to revoke consent at anytime.
In the form provided you had the option with what information you are comfortable with sharing with the school district, contact your teacher to change this.
